The Role

As Specialist Health Lead within the Camden Housing First service, you will pioneer a human-centred approach to health within homelessness services. You will work directly with people often overlooked by traditional systems, building trust, completing health assessments, designing personal health support plans, and acting as the vital link between individuals and the health support they deserve.

Key Responsibilities

  • Complete health assessments and design personal health support plans for clients
  • Lead multidisciplinary meetings and open doors to essential health services
  • Map local services, influence partnerships and drive more inclusive healthcare approaches
  • Build trust with clients who may feel wary of traditional services
  • Work within a psychologically informed, trauma-informed environment
  • Support safeguarding, record keeping, data and reporting requirements

Requirements

  • Knowledge of common health issues and barriers to care faced by people experiencing homelessness
  • Ability to build trust and engage clients who may be wary of services
  • Understanding of local health systems and ability to navigate them confidently
  • Experience or awareness of working with survivors of domestic abuse
  • Strong skills in support planning, safeguarding, advocacy and reporting
  • Ability to work both independently and as part of a team
